Preliminary Results of PERSONAE: A Personalized Internet-Based Cognitive Behavioral Therapy Treatment for Depression

Introduction: Depression is a globally prevalent psychiatric disorder, for which cognitive behavioral therapy (CBT) remains the gold-standard therapeutic intervention. In recent years, the delivery of CBT has increasingly been digitized, resulting in enhanced accessibility and treatment standardization through internet-based cognitive behavioral therapy (iCBT). However, depression is a heterogeneous condition characterized by substantial variation in symptom presentation, which may limit the suitability of standardized, linear iCBT protocols for all individuals.

PERSONAE is a personalized, internet-based CBT intervention delivered via a mobile application, designed to align treatment content and intensity with the individual symptom profiles and clinical needs of each participant. The intervention employs adaptive treatment strategies to optimize therapeutic relevance and responsiveness over time. Its efficacy and safety are currently being evaluated in a randomized controlled trial (RCT) in comparison with treatment as usual (TAU), standardized traditional iCBT.

Here we present preliminary findings from the first 25 participants enrolled in each trial arm.

Objectives: To investigate:
i) The preliminary results of the PERSONAE treatment compared to TAU, as measured by the Patient Health Questionnaire (PHQ-9).
ii) Trends in treatment adherence between the groups.

Methods: The first objective will be analyzed by comparing the change in PHQ-9 scores from pre- to post-treatment between the two groups using a linear regression model adjusted for pre-treatment scores.

The second objective will be analyzed by defining adherence as a binary variable (yes/no) determined by active engagement during the 12-week treatment period. A participant is considered adherent if they are active for at least 75% of the 12 weeks (with a minimum of one login per week) or complete at least 75% of the allotted exercises, which include the introductory session and eight subsequent steps (parts of modules). Adherence rates will be compared between the two groups using a two-sample proportion test.

Results: Analyses have not yet been conducted. Results will be available for presentation at the conference.

Conclusions: Identifying trends in the personalized treatment compared to TAU provides an initial indication of the potential benefits of personalizing interventions. It also offers insights into whether the treatment is as safe for patients as treatment as usual.
